Fillmore County Public Health works with individuals over age 65 and individuals with intellectual and physical disabilities to meet their health and safety needs. Public Health assists these community members by coordinating and managing services and providing referrals to service providers. These services and opportunities help clients to achieve their goals of living with greater independence. 

Fillmore County provides the following Home and Community-Based Services: 
  • Care Coordination
  • Long-Term Care Consultations and MnCHOICES Assessments
  • Long-Term Care Programs via Minnesota Waiver Programs
