Care Coordination

Fillmore County Public Health provides care coordination services to persons over age 65 who are enrolled in MSC+ or MSHO programs through BlueCross BlueShield, Medica, and UCare. Care coordination respects client independence and staff will work with you to create a plan that meets your needs and helps you be your healthiest.

Care coordinators review health care needs and assist members with:

Finding a specialist
Helping you learn about your medications
Providing information to help you and your family make health care decisions
Sharing information about preventing illness, accidents and trips to the hospital
Assistance with your care during and after a hospitalization
Answering your questions
